SAN ANTONIO -- The Nix Health Foundation Sports Medicine Center announces Saturday clinic dates. Middle school, high school, and college student athletes may be seen by volunteer physicians specializing in Orthopedic Surgery and Sports Medicine on May 14 and June 4.

At the Nix Health Foundation Sports Medicine Center, student athletes can receive complimentary evaluation of their injury. The Center is located on the 11th floor, suite 1116 of the Nix Medical Center at 414 Navarro in San Antonio. Registration is open from 6-9 a.m. and physician examinations begin at 7 a.m.

Each student should have the following information: their name, address, date of birth, Social Security number, home phone number, name of their school, athletic trainer’s or coach’s name, and a parent’s name, employer, and work phone number. Students should also have a parent, guardian, athletic trainer, or coach present with them at the clinic, copies of any X-rays or MRIs done before visiting the clinic, and proof of insurance, whether it is school or personal insurance. If the student has school insurance, they must have a school claim form signed by a parent.

For more information, call Jennifer Murphy at 210-579-3193 or visit www.nixfoundation.org/SportsMedicineCenter/index.asp.
