Floresville changes physical requirements
FLORESVILLE -- Beginning for the 2014-15 school year, athletic physicals are only required for incoming seventh-, ninth- and 11th-grade students, and/or students who answer “YES” to any of the first five questions on the “medical history form.” The remainder of the athletic paperwork must still be completed yearly, however.
A new option for completing the athletic paperwork is now available. This option is through an electronic form submission, and parents can connect to it through the Floresville athletic website at www.floresvilleathletics.us. Parents should click on the tab labeled “Rank1 Electronic Forms” from the athletics website.
Each required form is available to be completed and submitted online. Upon submission, parents will be emailed a copy of the form and will be notified by email as soon as the paperwork has been reviewed and accepted by the athletic trainer. This means no more handing in to coaches or the office, or dealing with lost forms. Once submitted electronically, a copy can be printed at any time for your records or reference. Full name, date of birth, and student ID number will be necessary to complete the electronic forms.
For those who are in need of physicals, they will be offered Thursday, May 15, from 5-7 p.m. at the north campus competition gym. The cost is $10 per athlete, or $20 per family.
