Health Insurance Marketplace requests verification
Important information for anyone who has been contacted concerning their Health Insurance Plan from the Marketplace has been released. This includes:
•Representatives from the Health Insurance Marketplace may call you or send you a letter if the information on your insurance application does not match official records.
•You may be asked to provide additional information regarding your dependents, family income, group health insurance, incarceration, or citizenship.
•These contacts and letters are not cancellation notices but you must respond, as requested, even if you already have a health-insurance plan.
•You may lose your tax credits or your health-insurance plan if the Marketplace is not able to verify the information on your application.
In order to assure continuing coverage through your Marketplace health-insurance plan and to continue to receive tax credits toward your insurance premiums:
•Understand what information is required in the notice.
•Gather the information and mail copies as requested in the notice.
•Include a copy of the notice that has the bar code marking; provide your legal name and application ID on each page you submit.
•You may also scan the information and upload it to your application at the Health Insurance Marketplace.
